Maxpro 20 mg is indicated for:

  1. Alleviating symptoms related to GERD, including persistent heartburn.
  2. Treating erosive esophagitis and maintaining its healing.
  3. Eradicating Helicobacter pylori infection in individuals with duodenal ulcer disease, in combination with amoxicillin and clarithromycin.
  4. Managing Zollinger-Ellison Syndrome.
  5. Treating duodenal and gastric ulcers caused by acid dyspepsia.

Dosage of Maxpro 20 mg:

  • Healing of Erosive Esophagitis: 20 mg or 40 mg once daily for 4-8 weeks, with the majority of patients healed within this timeframe. For non-responsive cases, an additional 4-8 weeks of treatment may be considered.
  • Maintenance of Healing of Erosive Esophagitis: 20 mg once daily (clinical studies did not extend beyond 6 months).
  • Symptomatic GERD: 20 mg once daily for 4 weeks, with the option for an additional 4 weeks if symptoms persist.
  • Helicobacter pylori Eradication: Triple Therapy—Esomeprazole 40 mg once daily, Amoxicillin 1000 mg twice daily, Clarithromycin 500 mg twice daily, all for 10 days.
  • Zollinger-Ellison Syndrome: Initial dose of 20-80 mg once daily, with individual dosage adjustments as needed.
  • Acid-related Dyspepsia: 20-40 mg once daily for 2-4 weeks based on response.
  • Duodenal Ulcer: 20 mg once daily for 2-4 weeks.
  • Gastric Ulcer: 20-40 mg once daily for 4-8 weeks.

Administration of Maxpro 20 mg:

  • Oral: Take one Maxpro 20 mg pill or capsule one hour before a meal, swallowing it whole.
  • Delayed-Release Oral Suspension: Mix entire contents of packet with 15 ml of water, stirring thoroughly, allowing to thicken for 2-3 minutes, then consume within 30 minutes. If using a gastric or nasogastric tube, administer with water similarly.
  • Injection: Administer 40 mg once daily by intravenous injection or infusion over specified durations. Avoid concurrent administration with other medications through the same intravenous site.

Interaction of Maxpro 20 mg:

Esomeprazole is primarily metabolized by CYP2C19 and CYP3A4 enzymes and may interact with drugs metabolized by these enzymes. However, clinically significant interactions with medications processed by other CYP enzymes are unlikely. Notable interactions include reduced clearance of diazepam and potential impact on medications sensitive to gastric pH changes.

Contraindications:

Maxpro 20 mg is contraindicated in individuals with known hypersensitivity to any of its formulations.

Side Effects of Maxpro 20 mg:

Common side effects include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth. Side effect occurrence during maintenance therapy does not significantly differ from short-term treatment.

Precautions & Warnings:

Patients should be aware that a positive symptom response to Maxpro 20 mg does not exclude the possibility of stomach cancer. It's important to take esomeprazole capsules at least an hour before meals, and for patients with difficulty swallowing capsules, pellets can be mixed with applesauce and ingested immediately. Antacids are acceptable during esomeprazole therapy.

Overdose Effects of Maxpro 20 mg:

Overdose symptoms are not well-documented, though acute toxicity has been observed in animal studies. Symptomatic and supportive care should be provided in case of overdose.

Storage Conditions:

Store in a dry place below 30°C, protected from moisture and light, and keep out of reach of children.

Mode Of Action:

Esomeprazole inhibits gastric acid secretion by specifically targeting the H+/K+-ATPase in gastric parietal cells. It demonstrates improved acid control compared to racemic proton pump inhibitors. Esomeprazole's pharmacokinetics include stable plasma protein binding, extensive metabolism by hepatic enzymes, and excretion via urine and feces.

Pregnancy:

Insufficient data exists on esomeprazole use during pregnancy. Nursing mothers should discontinue breastfeeding if esomeprazole is deemed necessary.

Pediatric Uses & Geriatric Use:

Safety and efficacy have not been established in pediatric patients. Geriatric patients generally exhibit similar responses to younger individuals, though individual sensitivity may vary. Dosage adjustments are not necessary for mild to moderate hepatic impairment, but caution is advised in severe impairment. Renal impairment does not significantly affect esomeprazole pharmacokinetics.
